What is the SCMA PCMH Initiative?

The SCMA Foundation is a nonprofit affiliate of the SCMA that for almost 40 years has, among other things, encouraged worthwhile professional activities in the delivery of medical services. The Foundation has received grant funding from the South Carolina Department of Health and Human Services to provide free educational, consulting, and coaching services to physician practices seeking recognition from the National Committee for Quality Assurance (NCQA) as a PCMH.

 

While these services will be offered to eligible practices across the State in any location, a special focus will be on physician practices in areas where health disparities in the population are especially pronounced. The SCMA will be hosting a regional educational seminar near you soon, and if you are already prepared for more intensive assistance, the SCMA is prepared to visit your office and work with you to achieve NCQA PCMH recognition. While an initial glance at NCQA’s standards for recognition may be intimidating, with your commitment and the SCMA’s assistance, physicians can anticipate a successful PCMH experience.

Scott Hultstrand is the SCMA's legal counsel and PCMH Project Director specializing in physician practice transformation. He is an NCQA PCMH Certified Content Expert, which is a credential indicating comprehensive knowledge of the NCQA PCMH requirements and application process, and the documentation of the NCQA PCMH Recognition Program.
